When your knee hurts on stairs or after a walk, being told to exercise can feel a little bit backwards. It is reasonable to want an explanation before adding more work to something that is already sore.

What does the evidence tell us?
A Cochrane review of knee osteoarthritis found that exercise can improve pain and physical function, but the size of the average benefit and the certainty of the evidence vary. The review also questioned whether some average improvements would feel meaningful to a patient. That is a reason to check your own response rather than promise a particular result. Cochrane review.
Your goals help give that response a practical meaning: being able to use the stairs at home, walk to the shop, or spend more time in the garden. A change in one of those activities matters alongside any change in a pain score.
Does discomfort mean the exercise is wrong?
NICE advises that discomfort can increase when someone first starts prescribed exercise for osteoarthritis, but an appropriate program still needs to be tailored and reviewed. You do not need to push through every painful response. NICE recommendations.
Tell your clinician what the discomfort feels like, when it appears and how the knee responds afterwards. Those details help them decide whether to adjust the work or reassess the problem; a blog cannot give you a personal pain limit.
What can physiotherapy add?
An assessment gives the exercise recommendation a starting point that fits you, rather than assuming that everyone with arthritis can manage the same work. It also gives you an opportunity to ask about activities you have been avoiding.
